Job summary

Upperline Health is seeking a Nurse Practitioner for their Palm Coast, FL clinic. This full-time position offers the opportunity to provide comprehensive clinic-based care for patients with chronic conditions as part of an interdisciplinary team. Qualifications

• A minimum of 6 months Nurse Practitioner experience in primary care/internal medicine.
• Value-based care experience preferred.
• Basic Life Support (BLS) certification required.

Responsibilities

• Develop clinical plans of care for adult patients with chronic conditions.
• Collaborate with the interdisciplinary team to meet patient needs.
• Educate patients about medication usage and diet.
