Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care at the Somerset Clinic, with potential home visits or telehealth. The role emphasizes complex, holistic plans of care for adults with chronic conditions, in collaboration with physicians and the broader care team.

Ideal candidates hold a Master's in nursing, board certification, and Florida licensure, and bring 2+ years in Primary Care or Internal Medicine.

Qualifications

• Master's degree in nursing required.
• Board certification in Adult, Adult-Geriatric or Family NP.
• Current Florida RN and NP license required.
• BLS certification required.
• 2+ years NP experience in Primary Care/Internal Medicine.
• Value-based care experience preferred.
• DEA registration or eligibility.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather history, exams, and diagnostics in clinic, telehealth, or home.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Educate patients and families on medications, diet, and prevention.
• Maintain medical records consistent with regulations and company standards.
• Participate in weekly team meetings and continuing education.
