Job summary

Upperline Health seeks a Nurse Practitioner for our Starke, FL clinic to deliver clinic-based and home-based care in a value-driven setting. You will develop care plans, diagnose, educate patients and families, and coordinate with the physician team and wider care network.

Ideal candidates hold a NP license, board certification, and have experience in primary care or geriatrics. This role offers a weekday schedule, competitive compensation, benefits, and CME support.

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
• Current State RN and NP licenses required.
• DEA registration or eligibility preferred.
• BLS certification required.
• Experience in primary care/internal medicine and value-based care environments preferred.
• Proficiency with AthenaHealth EMR and MS Office helpful.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, exam data, and diagnostics in home or clinic settings.
• Collaborate with interdisciplinary team to address physical and psychosocial needs.
• Prescribe medications as permitted by state Boards of Nursing.
• Provide telephonic support and participate in weekly team meetings.
• Maintain accurate medical records in accordance with state regulations.
