Job summary

Upperline Health is hiring a Nurse Practitioner (Adult/Adult-Geriatric/Family) to provide care in Citrus County, FL, collaborating with a local physician team to develop complex care plans. Care may occur in clinic, home, or via telehealth, aligned with the APP’s scope and patient needs.

Ideal candidates have primary care experience, compassion, and strong teamwork skills. The role supports weekday schedules with a comprehensive care team and value-based care delivery.

Qualifications

• Master's degree in nursing.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner.
• DEA registration or eligibility.
• BLS certification is required.
• Experience in primary care / internal medicine preferred.
• Comfort providing care in patients' homes.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather patient history, physical exam, and diagnostics in home or clinic.
• Develop and implement treatment plans aligned with patient goals and conditions.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and families.
• Educate patients and families about medications, diet, and crisis prevention.
• Coordinate care with other Upperline clinicians and community providers.
• Maintain accurate medical records per regulations and policy.
• Participate in weekly interdisciplinary meetings and continuing education.
• Prescribe medication as permitted by the state Board of Nursing.
