Job summary

Upperline Health Inc. seeks a Nurse Practitioner to provide clinic-based care at the Bradenton, FL clinic. In collaboration with the local physician team, NPs develop complex care plans, provide diagnostics and treatments in clinic or home settings, and educate patients on medications and prevention.

The role emphasizes value-based care and a weekday schedule. Ideal candidates have primary care/internal medicine experience, geriatric or home-based care exposure, DEA eligibility, and BLS.

Qualifications

• Master’s degree in nursing is required.
• Board Certification in Adult, Adult-Geriatric or Family NP is required.
• DEA registration or eligibility is required.
• BLS certification is required.
• Excellent communication skills (oral and written) are essential.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather history, perform exams, and diagnostic assessments in clinic or home settings.
• Create and adjust treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team to address physical and psychosocial needs.
• Educate patients and families on medications, diet, and prevention strategies.
• Coordinate with physicians and other providers to ensure continuity of care.
• Maintain accurate medical records per state and policy requirements.
• Participate in weekly team meetings and ongoing education.
• Provide telephonic support for escalated triage cases.
• Prescribe medications as permitted by state Nurse Practice Act.

Education

Master’s degree in nursing

Board Certification: Adult Nurse Practitioner OR Adult-Geriatric Nurse Practitioner OR Family Nurse Practitioner

DEA registration or eligibility

BLS certification
