Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Starke, FL clinic. In collaboration with Upperline’s local Physician team, APPs develop complex plans of care, provide medical diagnostics and treatments in the clinic, home, or telehealth, and educate patients and families on medication usage, diet & prevention.

Ideal candidates will have primary care experience and a patient-centered approach.

Qualifications

Master’s degree in nursing is required.
Board Certification in Adult, Adult-Geriatric or Family NP.
DEA registration or eligibility.
BLS certification required.
Experience in value-based care or home-based care is preferred.

Responsibilities

Develop and implement clinical plans of care for adults with chronic conditions.
Gather patient history, perform exams and diagnostics in home or clinic.
Develop and implement treatment plans aligned with patient goals and conditions.
Collaborate with the interdisciplinary team to meet physical and psychosocial needs.
Educate patients and families about medications, diet, and prevention.
Prescribe medications as permitted by the state Board of Nursing.
Maintain patient medical records per state regulations and policies.
Participate in weekly interdisciplinary team meetings and ongoing education.
Provide telephonic support for escalated calls from triage nurse.
