Job summary

Ideal candidates have a Master’s in nursing and board-certified NP credentials. DA and EMR experience (AthenaHealth) preferred; regular weekday schedule and telehealth options may be available.

Qualifications

• Master’s degree in nursing required.
• Board Certification in Adult, Adult-Geriatric or Family NP required.
• Minimum 6 months NP experience in primary care/internal medicine.
• Value-based care experience preferred.
• DEA registration or eligibility.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, physical exam, and diagnostics in home or clinic.
• Develop and implement treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide care in clinic and home settings with weekday schedule.
• Prescribe medications as permitted by the state Board of Nursing.
• Maintain patient medical records per state regulations and policy.
• Participate in ongoing education and team meetings.
