Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Kissimmee, FL clinic.

In collaboration with Upperline’s local Physician team, APPs develop complex plans of care in accordance with the patient’s health status and overall goals and values; provide medical diagnostics and treatments in the clinic, home, or telehealth, consistent with the APP’s scope of practice and the patient’s medical condition; educate patients and families on medication usage, diet &

Qualifications

• Master’s degree in nursing is required.
• Board certification for NP in adult/geriatric/family tracks required.
• DEA registration or eligibility preferred.
• BLS certification required.

Responsibilities

• Develop and implement clinical plans of care for adult patients.
• Gather patient history, physical exams, and diagnostics in home or clinic.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Prescribe medications as permitted by state Board of Nursing.
• Provide telephonic support for escalated calls from triage nurse.
• Document patient records per state regulations and policy.
