APPs develop plans of care, diagnose, treat, educate, and coordinate with an interdisciplinary team for adults with chronic conditions in clinic and home settings. Ideal candidates hold a master’s in nursing, NP board certification (Adult/Adult‑Geriatric/Family), and BLS, with experience in primary care or internal medicine and a comfort level for

Qualifications

Master’s degree in nursing required.
Board certification as Adult, Adult‑Geriatric or Family NP required.
Minimum 6 months NP experience in primary care/internal medicine.
Value‑based care experience preferred.
DEA registration or eligibility.
Basic Life Support (BLS) certification required.
Experience delivering care in patients’ homes.
Experience with EMR; AthenaHealth EMR preferred.
Excellent oral and written communication.

Responsibilities

Develop and implement clinical plans of care for adult patients with chronic conditions.
Gather patient history, perform physical exams, and diagnostics in home or clinic.
Develop and implement treatment plans aligned with patient goals.
Collaborate with interdisciplinary team to meet physical and psychosocial needs.
Educate patients and families on medications, diet, and crisis prevention.
Document patient records per state regulations and company policy.
Provide telephonic support for escalated triage calls and participate in weekly team meetings.
Prescribe medication as permitted by nursing board; engage in continuous education.
