Qualifications

• Master’s degree in nursing required.
• Board Certification as Adult, Adult‑Geriatric or Family Nurse Practitioner required.
• Current State RN & Nurse Practitioner license required.
• DEA registration or eligibility preferred.
• Basic Life Support (BLS) certification required.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, exam, and diagnostics in home or clinic.
• Collaborate with interdisciplinary team for holistic patient support.
• Educate patients and families on medications, diet, and crisis prevention.
• Prescribe medications as permitted by the state Board of Nursing.
• Maintain accurate medical records per state regulations.
