Job summary

ChenMed is seeking a Nurse Practitioner, Bridging Care Provider who will work with our clinical operations team to deliver direct patient care virtually and in home settings where appropriate. The NP will train new PCPs, provide bridging care in centers with limited staff, and manage patient assessments, histories, and care plans in accordance with ChenMed standards.

Qualifications

• ARNP or similar advanced degree in Nursing and current NP certification.
• Board certification by AANP or ANCC preferred for certain States.
• Possession of a DEA number may be required depending on State.
• Minimum 2 years of clinical experience as an NP in Family/Internal Medicine or Geriatrics.
• Experience as a preceptor or teacher preferred.
• BLS certification due within 90 days of employment.

Responsibilities

• Train new PCPs in market on our technology/EMR with RCCO guidance.
• Provide bridging care in understaffed centers, in clinic or virtually.
• Independently assess acute/non-acute problems; document and plan care.
• Develop patient care plans with attention to culture and family support.
• Collaborate with multidisciplinary team to improve care delivery.
• Prescribe medications per state regulations; participate in risk/quality activities.
• Perform clinical rounds and contribute to documentation standards.
• Additional duties as assigned by management.
