ChenMed LLC is seeking a Nurse Practitioner to train PCPs, provide clinic or virtual coverage, and independently manage patient care in a senior-focused network. You will work with the RCCO to enhance workflows and patient safety while meeting clinical metrics.

The role requires ARNP credentialing, state NP certification, and BLS; travel up to 75% is expected. Competitive compensation and comprehensive benefits accompany this position.

Formación

• ARNP or similar advanced degree in Nursing required.
• Current Nurse Practitioner Certification in the State of practice required.
• Board certification by AANP or ANCC is preferred but may be required for certain States.
• Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.

Responsabilidades

• Train new PCPs in a market on using our technology/EMR and provide bridging care in centers needing coverage.
• Independently assess acute and non-acute clinical problems and document patient management plans.
• Perform and document physical assessments, histories, and develop plans; counsel patients and families.
• Coordinate care and plan with RCCO and multidisciplinary team, including rounds and conferences.
• Prescribe medications per state regulations and refer for consultations as needed.
• Collaborate to analyze current health care delivery and participate in new practice patterns.
