Job summary

ChenMed is seeking a Nurse Practitioner for the Advanced Practice Provider, CareLine, delivering diagnostic care through virtual consultations and telephone. This role serves as the dispositional authority for after-hours and weekend calls, assessing and documenting patients’ health status and managing care accordingly.

The position requires NP board certification, BSN/MSN, and willingness to travel to Georgia or other states for onsite observation as needed, with a focus on telehealth practice

Qualifications

• Must have BSN and MSN-APN certification.
• Board certification by AANP or ANCC required.
• Compact multistate nursing license required.

Responsibilities

• Assess acute and non-acute clinical problems via telemedicine; document evaluations and management plans.
• Plan patient care based on population needs and protocol; act as patient advocate.
• Collaborate with PCPs and other providers to report health data and ensure guideline compliance.
• Ensure optimal outcomes through telemedicine and on-call PCP support when needed.
• Maintain professional relationships and use diverse formats to deliver care across the continuum.
