You will provide direct patient care across ChenMed centers, SNFs, and home settings, including geriatric assessments, history taking, exams, diagnoses, and plan of care. The role involves case management, documentation, and potential invasive procedures with competency. Travel up to 50-75% may be required depending on assignment.

Qualificações

• Assesses acute and non-acute clinical problems and documents histories.
• Plans patient care and develops nursing care plans.
• Performs patient management duties and coordinates referrals.

Responsabilidades

• Independently assesses acute and non-acute clinical problems.
• Plans patient care considering cultural background and patient needs.
• Writes admission, transfer and discharge orders and interprets studies.
• Prescribes medications per state practice and collaborates with physicians.
• Participates in rounds and communicates management strategies to the care team.
