Job summary

ChenMed LLC is seeking a Nurse Practitioner to join our clinical operations team, delivering direct patient care across ChenMed centers, SNFs, and home settings as assigned. The NP will perform comprehensive geriatric assessments, develop care plans, educate patients, and coordinate with physicians to ensure high-quality, evidence-based care.

The role requires certification, state NP licensure, and 2+ years in Family Medicine, Internal Medicine, or Geriatrics.

Qualifications

• ARNP or similar advanced degree is required.
• Current Nurse Practitioner Certification in the state of practice is required.
• Board certification by AANP or ANCC is preferred.
• DEA number may be required depending on state.
• Minimum 2 years of NP experience in Family Medicine, Internal Medicine, or Geriatrics.
• BLS certification within first 90 days of employment.

Responsibilities

• Independently assesses acute and non-acute clinical problems.
• Conducts physical assessments and patient histories.
• Develops and implements patient management plans.
• Orders and interprets labs and imaging as appropriate.
• Performs in-depth documentation and rounds with the care team.
• Collaborates with physicians and specialists to optimize patient outcomes.
• Prescribes medications per state practice laws.
