Job summary

ChenMed is seeking a Nurse Practitioner bridging care provider to deliver direct patient care across ChenMed medical centers virtually and in-home when assigned. Qualifications

• ARNP or equivalent advanced nursing degree and licensed NP in practice state.
• Current NP certification and BLS required; DEA may be required.
• Experience in family medicine, internal medicine or geriatrics preferred.
• Strong communication, teamwork, and teaching abilities.

Responsibilities

• Train new PCPs in a market on using our technology/EMR.
• Provide bridging care in centers needing coverage, clinic or virtual.
• Assess acute and non-acute problems; document history and plan.
• Develop patient care plans considering cultural background and patient support.
• Coordinate with multidisciplinary team to ensure effective care.
• Document progress notes, participate in rounds and conferences.
• Refer patients for consultations as indicated; advocate for patients.
• Recognize emergencies and initiate physician involvement as needed.
