Job summary

ChenMed is seeking a Nurse Practitioner to provide direct patient care across ChenMed centers, SNF, and home settings as needed. The NP will perform physical exams, develop care plans, educate patients, and coordinate with physicians and the care team to ensure quality outcomes.

The role requires ARNP or equivalent, NP licensure, and state-specific certification. Flexibility for travel and non-traditional hours is expected, with a focus on geriatric populations and a patient-centered approach.

Qualifications

• ARNP or similar advanced nursing degree required.
• Current Nurse Practitioner Certification in the State of practice required.
• Board certification by AANP or ANCC preferred for certain States.
• Current DEA number may be required based on State of practice.

Responsibilities

• Independently assesses acute and non-acute clinical problems.
• Writes admissions, transfers and discharges orders; manages patient care plans.
• Orders and interprets appropriate laboratory and diagnostic studies.
• Prescribes medications to patients based on state practice.
• Collaborates with physicians, nurses, PT, social workers and caregivers.
