Job summary

ChenMed is seeking a Nurse Practitioner/ARNP to join our Primary Care team. You will manage a patient panel, coordinate with care teams, and lead health‑promotion efforts across diverse populations.

This role emphasizes accountability, cost containment, and high‑quality patient care in a mission‑driven, physician‑led organization. The position requires an ARNP or equivalent, state certification, BLS, and potential DEA considerations.

Qualifications

• ARNP or equivalent advanced nursing degree is required.
• Current Nurse Practitioner Certification in the practicing state is required.
• Board certification by AANP/ANCC is preferred.
• DEA number may be required depending on state.
• Basic Life Support (BLS) certification within 90 days of employment.

Responsibilities

• Independently assesses acute and non-acute clinical problems and documents history, assessments, and patient plans.
• Plans patient care considering population, cultural background and support systems.
• Writes admissions/transfers/discharges orders and interprets labs/diagnostics.
• Refers patients for consultation as indicated and coordinates with multidisciplinary teams.
• Documents progress notes, supports care transitions, and participates in rounds and meetings.
• Demonstrates knowledge and communicates care strategies to improve patient outcomes.
