Job summary

Gilder Search Group seeks a Nurse Practitioner for a Skilled Nursing Facility (SNF) in Clemmons, NC. This full-time, in-person role runs Monday through Friday, 9 to 5, with no nights, weekends, or on-call, and focuses on primary care for long-term, geriatric patients.

Qualifications

• Active North Carolina NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.

Position Description

We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in Clemmons, NC. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call. As the primary care and rounding provider in the facility, you will manage a panel of long-term care and geriatric patients, working independently from facility staff while collaborating on patient care.
