Job summary

Gilder Search Group is seeking a Nurse Practitioner (NP) for a Skilled Nursing Facility (SNF) in Swansea, MA. This full-time, in-person role runs Monday–Friday, 9 to 5, with no nights, weekends, or on-call, and offers competitive pay and robust benefits.

Qualifications

• Active Massachusetts NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.

We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in Swansea, MA.
