Job summary

Gilder Search Group is seeking a Nurse Practitioner for our Skilled Nursing Facility in Fall River, MA. This full-time, in-person role offers a Monday-Friday schedule from 9 to 5, with no nights, weekends, or on-call.

As the primary care and rounding provider, you will independently manage a geriatric patient panel, perform assessments, adjust care plans, and document visits while collaborating with an interdisciplinary team to ensure high-quality patient care.

Qualifications

• Active Massachusetts NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.
