Job summary

Gilder Search Group is seeking a Nurse Practitioner for a Skilled Nursing Facility in Newburyport, MA. This is a full-time, in-person role, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call.

You will serve as the primary care and rounding provider for long-term care residents while collaborating with facility staff to deliver high-quality care. The ideal candidate has an active MA NP license and experience in geriatric care, with strong assessment and EMR skills.

Qualifications

• Active Massachusetts NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.
