Job summary

Gilder Search Group is seeking a Nurse Practitioner (NP) for our Skilled Nursing Facility (SNF) in East Boston, MA. This full-time, in-person role offers a Monday–Friday schedule, 9 to 5, with no nights, weekends, or on-call required, focusing on long-term geriatric care.

The NP will serve as the primary care and rounding provider, managing a panel of long-term care patients, while collaborating with the interdisciplinary team and facility staff to ensure high-quality care.

Qualifications

• Active Massachusetts NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.
