Job summary
Gilder Search Group seeks a Nurse Practitioner for a Skilled Nursing Facility (SNF) in Clemmons, NC. This full-time, in-person role runs Monday through Friday, 9 to 5, with no nights, weekends, or on-call, and focuses on primary care for long-term, geriatric patients.

Qualifications
- Active North Carolina NP license in good standing.
- Experience in skilled nursing, long-term care, or geriatric primary care preferred.
- Comfortable rounding independently and managing a full patient panel.
Responsibilities
- Serve as the primary care and rounding provider for patients in the facility.
- Conduct assessments, manage chronic conditions, and adjust care plans.
- Document visits accurately and maintain compliance with facility and regulatory standards.
- Participate in patient care meetings and coordinate with the interdisciplinary team.
