Job summary

We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in Wellsville, NY. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call.

As the primary care and rounding provider, you will manage a panel of long-term care and geriatric patients, collaborating with the facility team. Compensation ranges from $100,000 to $170,000 annually, with comprehensive benefits including medical, dental, vision, life

Qualifications

• Active New York NP license in good standing.
• Experience in skilled nursing or geriatric primary care preferred.
• Comfortable rounding independently with full patient panel.
• Strong assessment and documentation skills, EMR proficient.

Responsibilities

• Serve as primary care and rounding provider for facility patients.
• Assess, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain regulatory compliance.
• Participate in patient care meetings and coordinate with interdisciplinary team.
