Job summary

Gilder Search Group is seeking a Nurse Practitioner for our Skilled Nursing Facility in Henderson, NC. This full-time, in-person role runs Monday through Friday, 9 to 5, with no nights, weekends, or on-call, and offers competitive pay and robust benefits.

You will manage a panel of long-term and geriatric patients, provide primary care and rounding, and collaborate with facility staff while working independently from the facility team.

Qualifications

• Active North Carolina NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans; document visits accurately and maintain regulatory compliance.
• Participate in patient care meetings and coordinate with the interdisciplinary team.
