Job summary

Gilder Search Group is seeking a Nurse Practitioner for our Skilled Nursing Facility in Lexington, NC. This full-time, in-person role runs Monday through Friday, 9 to 5, with no nights, weekends, or on-call.

You will manage a patient panel of long-term and geriatric patients, working independently while collaborating with the care team. Applicants must hold an active North Carolina NP license, possess experience in skilled nursing or geriatric primary care, and be proficient with EMR

Qualifications

• Active North Carolina NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.

Compensation: $100,000 - $170,000 Annual Salary
