Job summary

Gilder Search Group seeks a Nurse Practitioner for a Skilled Nursing Facility in Stoughton, MA. This full-time, on-site role offers a Monday–Friday, 9–5 schedule with no nights or weekends, focusing on primary care for long-term and geriatric patients.

The NP conducts assessments, manages chronic conditions, documents visits in EMR, and collaborates with an interdisciplinary team while maintaining independence from facility staff to ensure high-quality patient care.

Qualifications

• Active Massachusetts NP license in good standing.
• Experience in skilled nursing, long-term care, or geriatric primary care preferred.
• Comfortable rounding independently and managing a full patient panel.
• Strong assessment, documentation, and EMR skills.

Responsibilities

• Serve as the primary care and rounding provider for patients in the facility.
• Conduct assessments, manage chronic conditions, and adjust care plans.
• Document visits accurately and maintain compliance with facility and regulatory standards.
• Participate in patient care meetings and coordinate with the interdisciplinary team.

We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in Stoughton, MA.
