Job summary

Mercy is seeking a Nurse Navigator to lead patient care coordination, assess needs, and guide care plans within inpatient and outpatient settings. The role emphasizes collaboration with bedside staff, discharge planning, and adherence to Mercy standards to ensure timely, safe, high-quality care.

The Nurse Navigator will educate patients pre-op, conduct daily rounds, and monitor LOS and utilization. A BSN is preferred, with required RN licensure and BLS; ACLS preferred.

Qualifications

• Current RN license in the state of practice.
• 2-3 years of relevant experience.
• BLS certification required.
• ACLS certification preferred.
• Graduate from an approved school of nursing.

Responsibilities

• Coordinate patient care plans with the bedside nurse.
• Perform pre-op education and daily rounding.
• Discharge planning and post-acute care coordination.
• Promote quality patient care and adherence to Mercy standards.
