Job summary

Mercy in Missouri is seeking a Nurse Navigator to lead care coordination for inpatients and outpatients. You will assess care needs, guide teams, and ensure timely, safe, high-quality care while aligning with Mercy standards.

Responsibilities include pre-op education, daily rounding, discharge planning, LOS management, and post-acute care coordination. A BSN is preferred and an RN license in the state is required, with 2–3 years of experience and BLS; ACLS preferred.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• 2-3 years’ experience required.
• BLS certification required.
• ACLS preferred.

Responsibilities

• Coordinate patient care plans for inpatient and outpatient populations.
• Provide pre-op education and daily rounding.
• Lead care coordination and ensure adherence to Mercy standards.
• Discharge planning and post-acute care coordination.
• Monitor LOS and utilization management.
