Qualifications

• Active RN license in the state of work.
• BLS certification from AHA or Red Cross.
• Minimum 1 year inpatient nursing or hospice experience for external candidates; 6 months LPN with hospice/home health for internal.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and insurance; reliable transportation.

Responsibilities

• Provide direct care to patients and families as part of the IDG.
• Coordinate the Plan of Care reflecting patient needs.
• Educate patient and family about disease, palliative care, safety.
• Communicate changes in patient condition to IDG and physicians.
• Assist with admissions, orders, documentation and discharge planning.
• Support bereavement interventions with psychosocial staff.
• Supervise other nursing staff as needed.
