Job summary

Chapters Health is seeking a Nights & Weekends Registered Nurse to provide patient visits with emergent, critical needs in home care and facility settings, collaborating with the IDG to implement a Plan of Care and ensuring timely, evidence-based nursing care.

The role requires active RN licensure in the state of employment, BLS certification, and ability to manage the emotional stress of caring for terminally ill patients. A flexible schedule and reliable transportation are essential.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG).
• Coordinates the Plan of Care, ensuring an individualized Plan of Care reflecting evolving needs.
• Educates patient, family, caregivers and other health professionals about disease, palliative care and safety practices.
• Reports changes in the patient’s condition to IDG or other health professionals.
• Supervises patient care given by other nursing team members (LPNs, Hospice Aides, contracted nurses).
• During emergencies, may be required to report to a designated location to ensure continuity of services.
