Job summary

Qualifications

• RN license in the state of employment.
• Minimum 1 year of nursing experience; hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Clear English communication (spoken and written); Spanish a plus.

Responsibilities

• Make after-hours visits to patients in home or facility settings.
• Assess patient/family needs and determine appropriate nursing interventions.
• Collaborate with IDG and other health professionals for care planning.
• Document per policy and maintain patient records.
• Provide compassionate customer service to patients and families.
