The ideal candidate holds an active RN license, BLS certification, and prior hospice or inpatient nursing experience, plus a valid driver's license and reliable transportation.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals.
• Minimum 1 year of inpatient nursing or hospice experience; internal applicants require 6 months of LPN with hospice/home health within an affiliate.
• Valid driver’s license and automobile insurance per company policy.
• Experience with an EMR/EHR system.
• Ability to communicate effectively in English (verbal and written).

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient, family, and caregivers about disease process, palliation, caregiving, dying process and safety practices.
• Reports changes in patient condition to IDG or other health professionals.
• Attends patient deaths and prepares body for transport; oversees medication disposal.
• Communicates with physicians, staff, patients, families, and supervisors; uses SBAR.
• Supervises care provided by LPNs, Hospice Aides, and contract nurses.
• May be required to report to a designated location during emergencies; may include overnight stays.
• Performs other duties as assigned.
