Job summary

Chapters Health System in Florida is seeking an RN for Nights and Weekends to perform after-hours home visits to patients, families, and community partners, building trusted relationships and delivering compassionate care. The role requires an active RN license, 1 year nursing experience (hospice or hospital preferred), EMR/EHR experience, and strong communication.

You must have a valid driver's license and reliable transportation to cover assigned territories.

Qualifications

Current license as RN in the state where the employee will be working
Minimum of one (1) year nursing experience; hospice or hospital experience preferred
Previous experience with EMR/EHR (Electronic Medical/Health Record) system
Ability to communicate in English; bilingual Spanish preferred
Mobile Driver - valid license and automobile insurance per Company policy
Reliable transportation
Excellent critical decision making and problem-solving skills
Strong clinical and assessment skills
Ability to use equipment with visual/auditory mechanisms
Excellent time management and prioritization
Ability to handle stressful situations professionally with customer service
Strong information seeking / probing skills

Proficient in MS Office and online navigation
Available for extended hours and shifts as scheduled

Active BLS certification

Responsibilities

Makes after-hours visits to patients in their home, assisted living, nursing home, or hospital within the assigned territory
Assesses patient/family needs and provides nursing interventions by phone and during visits
Delivers direct care within IDG framework including psychosocial, spiritual, cultural, physical and biological components
Collaborates with professionals to promote quality and continuity of care
Educates patient/family and other health professionals about disease, palliation, and safety
Reports changes to IDG and ensures timely follow-up with Admissions
Maintains documentation per policy; uses SBAR for updates
Supports after-hours staff during emergencies and adapts to urgent needs
Completes other duties as assigned
May participate in provider referrals and discharge planning
Supports quality improvement activities
