Chapters Health System in Virginia is seeking a Per Diem RN to assess patient and family needs, coordinate care, and deliver palliative and supportive services within the IDG framework.

The role requires an active RN license, BLS certification, and strong communication skills; eligible candidates will have hospice or hospital experience and be comfortable with EMR/EHR systems and driving to patient visits. Flexible scheduling is available.

Formación

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one (1) year nursing experience; hospice or hospital experience preferred.
• Experience working with an EMR/EHR (Electronic Medical/Health Record) system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet Hospice House patients’ needs.
• Ability to manage the emotional stress of caring for terminally ill patients and families.
• Ability to perform essential functions including lifting patients/equipment.

Responsabilidades

• Provides and manages direct care to patients and families as part of IDG with appropriate nursing interventions.
• Coordinates the Plan of Care and ensures it reflects evolving patient needs.
• Educates patient/family on disease, palliative care, and safety practices.
• Reports changes in patient condition to IDG or other health professionals.
• Assists with admissions, orders, and coordination for discharge.
• Performs documentation per Company policy and SBAR communications.
• Maintains professional relationships with facilities, hospitals, physicians, patients and families.
