Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS certification for healthcare professionals.
• Minimum one year nursing experience in hospice or hospital settings preferred.
• Experience with EMR/EHR systems preferred.
• Valid driver’s license and automobile insurance per policy.

Responsibilities

• Provide direct care to patients and families as part of the interdisciplinary group (IDG).
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patients and families on disease processes and palliative care.
• Document thoroughly per policy and communicate with physicians and staff.
• Perform admissions, triage, and ensure safe discharge when appropriate.
