Chapters Health System is seeking a Per Diem RN to assess patient and family needs, coordinate care with the IDG, and provide palliative and supportive nursing services across hospice settings.

The role requires an active RN license, BLS, and hospice or inpatient nursing experience. Strong communication in English and ability to handle emotional stress are essential, with a flexible schedule to meet patient needs.

Qualifications

• Active RN license in the state of employment.
• BLS for healthcare professionals (American Heart Association or Red Cross).
• Minimum 1 year inpatient nursing or hospice experience; internal applicants: 6 months LPN in hospice/home health within affiliate.
• Experience using an EMR/EHR system.
• Valid driver’s license and automobile insurance per policy; reliable transportation.
• Ability to communicate effectively in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to manage emotional stress when caring for terminally ill patients and families.
• Ability to physically access patient rooms and navigate stairs/limited spaces within the service area.

Responsibilities

• Provide direct patient and family care as part of the Interdisciplinary Group (IDG).
• Coordinate Plan of Care reflecting evolving patient needs.
• Educate patient and family on disease, palliative care, and safety practices.
• Document and report changes to IDG and other health professionals.
• Perform nursing interventions to alleviate symptoms and maximize comfort.
