Responsibilities

• Provide direct care to patients and families as part of the IDG.
• Coordinate Plan of Care reflecting patient needs.
• Educate patient/family on disease, palliative care, safety practices.
• Communicate changes in condition to IDG and physicians.
• Support timely admissions and appropriate level of care.
