Chapters Health System in Florida is seeking an RN for Nights and Weekends to perform after-hours visits to patients in their homes, facilities, and with community partners. This role emphasizes compassionate care, coordination with the IDG, and timely documentation.

The position requires an active RN license in Florida, 1 year nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system.

Responsabilités

• Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate territory.
• Provides compassionate customer service to patients and families.
• Documents per policy and communicates with the IDG and other health professionals.
