Job summary

Gentiva is seeking an Afterhours RN Care Manager to deliver high-quality hospice care during nights, weekends, and on-call hours. You will assess patients, coordinate care with the IDG, and ensure compliance while maintaining detailed documentation.

The role requires an active RN license, CPR, and a minimum of one year in hospice, acute care, oncology, or home health. Expect a fast-paced, compassionate team environment with comprehensive benefits.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.
• Active CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Provide direct in-person patient care during after-hours, weekend, and on-call shifts.
• Assess and coordinate patient and family needs during after-hours.
• Maintain primary clinical oversight and care plans with physicians and IDG.
• Document assessments, plans of care, and visits.
• Support regulatory compliance and survey readiness.
