Job summary

Gentiva Hospice is seeking an After Hours RN to provide direct patient care to hospice patients and support to families after normal business hours on an on-call basis. You will manage an interdisciplinary team and coordinate care, documentation, and case management of the patient’s records.

Responsibilities include initial and ongoing patient assessments, educating families, and ensuring adherence to the Plan of Care, quality standards, and regulatory requirements while maintaining flexibility

Qualifications

• RN required with knowledge of hospice and terminally ill patient care.
• CPR certification required and ongoing competency validation.
• Licensed to practice in the state of employment, with continuous onboarding and training.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop and implement the Hospice Plan of Care in partnership with the Interdisciplinary Group.
• Educate patients and families on care plans, medications, and symptom management.
• Maintain accurate medical records and ensure compliance with policies and regulatory requirements.
• Coordinate care across the hospice team and monitor staffing and scheduling.
