Job summary

Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it’s needed most. You’ll report to the Administrator or Patient Care Manager and work from patients’ homes or facilities, coordinating with daytime teams.

You must hold a current RN license in the state, have at least 1 year of nursing experience, and be CPR certified.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• A hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide direct patient care after normal business hours.
• Serve as liaison between patients, families, physicians, and hospice team.
• Respond promptly to urgent calls and symptom management needs.
• Evaluate patient and family needs to guide care and implement the plan of care.
• Document visits and communications in the medical record.
• Collaborate with interdisciplinary team including LPNs/LVNs, CNAs.
• Maintain continuous access to TeleMed app during shifts.
• Maintain safety and infection control practices.
