Job summary

Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct patient care, coordinate with families and the hospice team, and document clinical visits in the EMR while maintaining continuity of care.

This role requires an active RN license, CPR certification, and at least 1 year of nursing experience. Driver's license and reliable transportation are essential for home visits across assigned regions.

Qualifications

• Current RN license in the requested state (or eligibility to obtain).
• 1+ years nursing experience.
• Valid driver's license and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Serve as primary liaison between patients, families, physicians, and hospice team during shifts.
• Document visits and communications accurately in the EMR.
• Provide direct patient care and support after normal business hours.
• Care coordination with interdisciplinary team for pain and symptom management.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with the daytime care team.
