Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide compassionate hospice care outside standard hours. This role ensures continuity of care, coordinates patient needs, and maintains clinical oversight during nights, weekends, and on-call shifts.

You will conduct assessments, support the interdisciplinary team, and educate families on disease progression, symptom management, and hospice philosophy in a fast-paced after-hours setting.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person care during after-hours, weekend, and on-call shifts, including assessments and visits.
• Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.
• Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members.
