Job summary

Gentiva is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard hours. You will provide direct patient care during nights, weekends, and on-call periods, while maintaining primary clinical oversight for assigned patients.

You will collaborate with physicians and the IDG, conduct assessments, educate families, and ensure accurate documentation and compliance across after-hours operations.

Qualifications

• Must have active RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Coordinate care with physicians and IDG; maintain plan of care updates.
