Job summary

Caretenders Hospice, a part of LHC Group and part of the Optum family, seeks an in-home Home Health Aide to assist with personal care and daily living activities for patients at home in Ohio. This PRN/inconsistent position involves in-home hospice visits within a 50-mile radius and requires an OH STNA certification and reliable transportation.

Responsibilities include monitoring patient condition, documenting vital signs, assisting with bathing and mobility, and ensuring safety.

Qualifications

Current Ohio STNA certification is required.
Valid driver’s license and reliable transportation.
Travel within a 50-mile service radius for patient care.

Responsibilities

Provide personal care and assist with daily living activities including bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
Ensure a safe environment and proper operation of medical equipment; respond promptly to client needs.
Serve clients regardless of age, race, creed, color, sex, disability, or financial condition.
