Job summary

Powerback Rehabilitation To You seeks a licensed Speech-Language Pathologist to provide in-home outpatient services for older adults. The role includes evaluating and treating communication and swallowing disorders in patients’ homes or in facilities, coordinating care with the rehab team, and ensuring comprehensive documentation.

A valid state license and ASHA eligibility are required, with supervision of trainees as applicable.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice.
• Must possess or be eligible for a Certificate of Clinical Competence from ASHA or be eligible for ASHA CFY.
• Must have a valid driver’s license or reliable transportation to patients’ residences.

Responsibilities

• Provide outpatient rehabilitation services in home-based and community settings.
• Evaluate, diagnose, and plan therapy for communication and swallowing disorders.
• Coordinate with interdisciplinary rehab team and physicians to optimize patient care.
• Direct patient participation in therapy tasks and adapt plans of care as needed.
• Maintain documentation and comply with scheduling, billing, and departmental requirements.
• Supervise CFYs, students, and support staff as required by licensure.
