Job summary

Powerback Rehabilitation To You is seeking a Speech Language Pathologist to provide in-home outpatient rehabilitation services for older adults, with the flexibility to work across patients in homes and living facilities. The role emphasizes patient-centered communication goals, evaluation, and alignment with physician directives.

You will collaborate with a multidisciplinary team, document progress, and adapt plans of care as needed to maximize functional communication and swallowing outcomes

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from the patients' residence.

Responsibilities

• As the Speech Language Pathologist, you will primarily provide outpatient rehabilitation services in an in-home setting for older adults, including homes and senior living facilities.
• Develop and implement speech-language treatment plans, assess progress, and document outcomes.
• Coordinate care with the rehabilitation team, physicians, and families to ensure patient-centered goals.
• Adapt plans of care as needed and promote safety and best practices in the home environment.
• Perform routine reexaminations and adjust therapy to achieve functional communication and swallowing goals.
• Support CFYs, students, and staff in accordance with licensure requirements.
