Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist to provide in-home outpatient therapy for older adults, in patient residences or facility settings. You will evaluate and treat communication and swallowing disorders, tailor plans of care, and document progress with a flexible schedule.

Must hold a valid SLP license in the practice state and have reliable transportation. Prior supervision of CFYs and other team duties may apply; opportunity for professional growth, CEUs, and visa

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from the patients' residence.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults or in facilities as appropriate.
• Supervise CFYs, students and support personnel where applicable per licensure requirements.
• Adhere to schedule, complete documentation and attend care conferences.
• Collaborate with rehabilitation team and physicians to select the best therapy plan.
• Assess patients, develop goals, implement treatment plans, and document progress.
